In order for us to make an informed decision and determine what assistance may be appropriate in your circumstances, we kindly request that you complete and submit the Financial Hardship Application along with supporting documentation. These supporting documents may include but is not limited to:
Three months of recent bank statements
Three months of payslips
Centrelink Summary Statement
Rent/mortgage Summary and Statements
Proof of other outstanding debts, bills, etc.
